Team repository error while creating issue from Risk

Hi ,
I am trying to create issue work item from Risk. I have chosen option to create issue from risk and trying to save work item but its througing an error...
Error Details: Team Repository Error: An unknown error has occurred in team repository connection.

I assume this is from the eclipse client, since you are getting an error with the team repository connection. Do you get an error from the web UI?

We need some more data:
1. What kind of client do you use (Eclipse client, web ui)?
2. Can you provide screen snapshot?
