Insight Trace reports do not show linkage when requirement linked to script and then u to test case.

I have linked our requirements to specific steps in the Test Scripts (TS).  I've then linked the Test Script to the Test Case (TC) and indicated that I want the TS-linked requirements to also be included in the TC.  When I run the Insight Trace reports (or any other reports which should show linkage) it does not show the requirement links to the TCs (and there is no means by which Insight shows the TS link).  We have Insight 1.1.1, Using CLM 4.0.0.1.
