Collisions Detected Error With Lifecycles?

I created a master lifecycle and was deploying it in several communities. It worked fine for the first 7 or so communities and then I started getting an error message saying "collision detected on insert of lifecycle".

The debug log has a lot more lines, but that is the gist of it.

What causes this, and more importantly how do I fix it? There are no other lifecycles defined yet in the communities where I am getting this error.
