Strange Behavior on Policies

Sometimes between one state and other, the policies configured on the states show different icons.

The normal icons are a check when all policies are OK, a yellow triangle when any policy return
a Warning or a red x when any policy fail.

I can't identify the reason but sometimes a right red arrow icon appears aside of the
policies' results. What it means?

When a policy does not runs in a event, this icon appears.

If a lifecycle has two policies configured, the first to be fired on the asset save and the other on the state change. If a asset is only saved, the second policy shows this icon, meaning that this policy exists but not runs.
