RQM 6.0.6: Error message while saving test case

"The Test Case cannot be saved.
Validation Errors for item: type= testCase, itemid= [UUID_xitfoBQZEEM3nKShu....]
Duplicate item references are not allowed: collections"
What does this error mean? What is the problem? Do I need to create a Case?
