RTC allows the same precondition or follow-up action to be added to an operation more than once. Is there a technique when writing a precondition or follow-up action to only allow it to be added to an operation once? Specifically, once per operation/role.

No, there's no mechanism for enforcing this. If this is a problem, you may need to look at your pre-conditions and make sure they are coded to the specifications, they should not have side-effects.
