 I want to send a reminder mail automatically to the approver if the approval is pending in a work item more than 5 days .

This is something you would have to do using a custom extension. Here is an example that might help: http://jorgediazblog.wordpress.com/2013/06/28/rtc-custom-scheduled-async-task-example-explained/

As a side note, if you agree that spamming users inbox with all these messages is not a good idea, you could try to use a query based dashboard. i am not sure if you can model 5 days overdue with queries though.
