how can we report the approvals, dates, approvers for all the code reviews in this release (as a delta from last release)?

we are running on 4.0.4, insight being installed (slowly) as we speak.

we need a report (that we can save) that documents all the code reviews of all the changes in all the builds since a previous (arbitrary prior) release (build).

query can give me approvals by Bob, but I cannot get all approvals and sort them by approver, and date/time

(cause I can't get the approval info as output of the query).

anyone have any suggestions?
