How do I add a follow up action to multiple existing projects?

I have written a custom process participant that can be applied as a follow up action.  I would like to apply the participant to all of the existing projects on my server.  Is there a way to apply the participant to all projects instead of manually applying the participant to each project individually?

sadly no.

you could write a tool to update the XML on the client process areas, but there is nothing out of the box to do that.

this is why shared project areas are so important

Yes, unfortunately there is no way (I am aware of) to activate it automatically for multiple projects. You have to configure it manually for each. There are probably reasons for this as well One could be that they often rely on input that is dependent that is specific to the project area or the underlying process.
