Hi,
the download page https://jazz.net/downloads/rational-team-concert/releases/4.0.0.1 has several tabs. The release notes and New&Noteworthy tabs have links to other products and also links to work item queries for a detailed look at the PMR's and work items addressed with the fix pack.
There is also the Upgrading tab https://jazz.net/downloads/rational-team-concert/releases/4.0.0.1?p=upgrading that tells how to upgrade (I would also always check the online help).
