How should I differentiate my bunch of baselines to deliver to their respective streams in RTC SCM?

There really is not enough information with this question to answer it adequately. Ex: What are the real issues/difficulties are you facing?
Only the developers / team leads would know which baselines should be delivered to which streams.
Perhaps a baseline naming convention might help (the name of the baseline can be changed after it has been created).
