Null error when importing spreadsheet

 I am attempting to import a csv into an existing module.  I get hundreds of "null" errors on just about every line.  In the past, it's the Name field that has "un-bound" and when you redo it, the import works.  However, it's just not working.  Any ideas? It seems the Name field unbinds whenever it feels like it and causes trouble.  This also happens in change sets and when you deliver them, the primary text doesn't get updated.  I have to go through every single module and redo the name fields constantly.
