RTC E-signature pop-up box?

I've added a precondition in RTC that requires an e-signature for approvals. I've added e-signature as a custom attribute for that work item type and successfully added that attribute to the editor presentation. Trouble is, the e-sig box shows up as a string line without a prompt for entering the password.

How can I setup the e-sig process so that the original log-in user name and password pop-up box appears for the e-signature requirement?

I am also not getting any pop-up box for e-signature.
