review the work item editor presentations for work packet and task.

appears to me that some attribute presentation is not configured correctly and has to be either corrected or removed altogether....

some other way a work item attribute presentation could lose its composure is due to an attribute delete.  Running "Check Attributes usage in Repository" would be advisable in that case.  If this does not fix it automatically, the presentation corresponding to the deleted attribute could be removed manually.

if you were working directly with configuration source (not recommended), possible that accidentally a attribute associated to a presentation is gone missing.

If this problem surfaced recently(if you have a reference to some point in time), you could look at the configuration source history (Eclipse Client > Open the PA's Configuration Source Tab > Right anywhere in the source and pick Show History), from the history, you can perform a compare on a previous history and from the differences shown you should be able to figure out what is gone missing...

hope this helps...

if the issue prevails, i would suggest raising a ticket to support with the reproducible process template shared to them for further investigations.
