RTC Access

How granular is security/access in RTC v4.0? Meaning as CM I need access to work items, get latest source code to sync working directory for builds/compile but should not be able to actually update/delete/add code.

It appears from what I read since I posed this question one can manage permissions at the lifecycle project level.

Read access and write permissions are managed separately in RTC, so a configuration like the one you describe is very possible. The ability to "update/delete/add code" would be managed by controlling who has permission to deliver change sets in RTC's source control system.
