How do I move from a Jazz Team Server 6.0.2 single server topology to a Jazz Team Server 6.0.2 enterprise topology?

We currently are running Jazz Team Server 6.0.2 as a single server - MS Server 2008 R2- running CCM, RM, QM, DCC, RELM, RS, GC, LDX, JTS, and LQE databases. We are using DB2 Workgroup and the Liberty bundled version.
We also have DOORS classic and DWA running on this server using Tomcat. This topology was selected because this was to be a small prototype. Well, lots of people like it and want it so the databases have become huge and we are having trouble with native memory exhaustion, etc.

So we ordered 2/new servers w/MS Server 2012 R2 and 4/cores and 2/copies of Server 2012 R2 in order for us to have 4/VMs per server - Hyper-V, plenty of memory and 23 SSD drives. We want to set up an enterprise topology using the Bundled Liberty and DB2 Workgroup database sw. I also need to know if the enterprise topology requires SSO and LDAP. I am new to installing the enterprise topology. Any help would be greatly appreciated.

As a rough summary
  • You set up a reverse proxy that hosts the public URI and make it work.
  • You backup your applications (config files, indexes, db,...). You especially backup the applications you want to move to another box.
  • You undeploy the applications to move
  • You install the application server and the applications on the other box, setup the proxy forwarding and restore the configurations and deploy them
