When you install the RQM Excel/Word utility, it comes with many samples that can be imported; for Excel they are located at:

…..\IBM\RQMExcelImporter\Samples

I would take a look at :

TestPlan with TestCases 001.xls

Testplan_Testcase_Testscript_TER_Environment_Iteration_AllInOne.xls

Both of these examples should help you get started

I am able to upload, but the issue is the test script name is coming as first step in the steps of the test script.
