How can I move data from a sql server for 4.0.3 to a new sql server running 5.0?

We are migrating from Jazz 4.0.3 to 5.0.  New servers have been set up and Jazz 5.0 runs fine.  Now I need to transfer the data.  There doesn't seem to be steps to do that.  Has anyone done this?  Surely someone out there has migrated from 4.0.3 to 5.0
