How do you modify an overdue approval if you cannot see it in the approvals tab?

I was subscribed to an approval on a work item that had a due date assigned however I keep getting emails that it's overdue but the approval is not there to change.
I see in the work item history that an approval was set with a due date, but on the "Approvals" tab it's just not listed.
Has anyone run into this or have any tips on how to repair it?

Web or eclipse ui? What version?
I cannot see the approval in either (UI/Eclipse). The person who created the work item is an archived user. On the approvals tab I do not see the approval, but can see it under the "History" tab
Here's the message/email I receives:
Subject: Overdue 'Review' request: 'Review of ant file' (work item 5215)
ID: 5215 Open in web browser
Type: Enhancement
Summary: Rework ant build file in order to this product using source management
Team Area: Team - Java API
Filed Against: Java category
Status: Ready for QA
The 'Review' request 'Review of ant file' is overdue since June 1, 2012
Right now, I believe I am at 4.0.1 but I will be checking that once I am at the machine again.
