Can't create or view one of our work item types in the web interface after upgrade to 4.0.4

Hi all,

At the weekend, we upgraded our server from 4.0.2 to 4.0.4

We can now no longer view or create one of our custom work item types called a 'change' using the web interface.  I have tried doing this in both FF 17.0.9 ESR and IE 8 on a WinXP client.

We can still view/create this work item type using the Eclipse client.

Out server uses tomcat, db2 9.7 and runs on RHEL 5.x

When trying to create a 'change', the 'Work Items' menu gets a star next to it and you can see the newly created work item listed in the unsaved work items section but you get a blank page.
In ie8 we sometimes get a 'TypeError: Type mismatch' error and the status bar at the bottom reports an error:

I think this may be the same problem we faced recently.

Do you see the above mentioned message (especially when stepping through a displayed query list ) ?

In this case, the Editor Presentation had a presentation for "Relationship" (e.g. some kind of link) that was not on the Links tab of the presentation.   The work around was to uncheck the "Hide if Empty" on that presentation.  It was also suggested that the label be hidden.
