 Rich,
The RFT adapter is included with the install bits of RFT
You can access it through the start menu 
IBM Software Delivery Platform -> IBM Rational Functional Tester -> Adapter for Rational Quality Manager Integration
You'll have two options:
Configure Adapter
Start Adapter

I'll need to find about about the QTP adapter

 For the QTP adapter:
Downloads -> RQM 4.0.2 -> All downlolads tab -> (Under the Extras heading) Rational Quality Manager HP Quick Test Professional Adapter
